High School Athletic Checklist
High School Athletic/Activity Contract and Physical and Eligibility Form *
- Insurance section of form is complete (Medical Insurance is mandatory to participate in sports.
- Medical Insurance can be purchased through the Tacoma School District. Please contact your building Athletic Director if this service is needed.
- Student and Parent Signatures are complete
- Physical Exam is complete and signed by the Physician. Physicals are good for 13 months in the Tacoma School District.

- Form is complete
- Parent Signature is complete
ASB Card Purchased
Cost of ASB is $30.00

* Forms are usually available for pickup at the office so you can have them all ready at Tiger Day - getting cleared = submitting all completed forms to athletic director Catalinich - this must happen before you can participate in tryouts.
